2UP AYUSH Counselling Allotment Letter (Original to be submitted)
310th Mark sheet (Copy, original will be verified and returned)
410th Passing Certificate (Copy, original will be verified and returned)
512th Mark sheet (Copy, original will be verified and returned)
612th Passing Certificate (Copy, original will be verified and returned)
7Transfer Certificate/School Leaving Certificate & Character Certificate (Original)
8Migration Certificate (Original; required for students from outside Uttar Pradesh)
9Aadhaar Card/Govt. ID (Copy; original for verification)
10At least 8 passport-size self-attested photographs
11Anti-Ragging Affidavits (Student & Parent separately) on ₹10 Stamp Paper (after admission)

Steps for Admission:

  1. First Register through by NEET 2025-26 Rollnumber.
  2. Fill Application Form through login by mobile No.
  3. Pay Fees after application filled Successfully.
  4. Verification of marksheets and documents (submit in serial order, check online form & fee).
  5. Issue of Provisional Admission Letter from Principal.
